Documents Required, Non-returnable (For Working Adults):

A photocopy of the NRIC (both sides) of the applicant and;

1. For Salaried Employees:

• Latest computerised payslip OR
• CPF Contribution History Statement (for past 12 months) OR
• Latest Income Tax Notice of Assessment AND CPF Contribution History Statement (for past 12 months) OR Latest computerised payslip

2. For Self Employed:

• Latest 2 years Income Tax Notice of Assessment AND
• Bank statement (for past 3 months)

3. For Commissioned or Variable Income Earners:

• Latest 2 years Income Tax Notice of Assessment OR
• CPF Contribution History Statement (for past 12 months)

Documents Required, Non-returnable (For Tertiary Education Students):

• A photocopy of NRIC (both sides) of the applicant and;
• A photocopy of applicant's Student ID Card/ Student Card / Matriculation Card (front side)

Documents Required Non-returnable (For NSFs):

• A photocopy of the application's SAF / SPF / SCD 11B Identification card (both sides)

For Applicants Below 21 Years Old:

• A photocopy of the NRIC (both sides) of the applicant's parent / guardian
  

Notes:

The applicant's parent / guardian must be a Singaporean or Singapore PR. The addresses for both the applicant and their parent / guardian must be similar for verification purpose.
